U-TuRN
The Iowa State Translational Research Network (U-TuRN) is a network of transdisciplinary researchers who work to enable healthy lifestyles by empowering communities to take science informed action. By not only building and supporting the infrastructure needed for community and campus partnerships, but also evaluating and addressing the challenges that limit the adoption, implementation and sustainability of these partnerships U-TuRN works to impact lasting change in local communities. For more information on U-TuRN and their contribution to Walk with Ease in Iowa, click here or visit https://www.uturn.iastate.edu/project/walk-with-ease/ Iowa Community Hub and CHPcommunity
The Iowa Community Hub is a community care hub led by a non-profit group called CHPcommunity. The HUB works with network partners to connect Iowans with meaningful evidence-based programs. By serving as a "one-stop-shop" for identifying individuals, connecting them with appropriate programs, and supporting sustainable programming by facilitating data collection and providing training and support to community partners, the HUB helps avoid duplication of effort, and prevents individuals from falling through the cracks. Organizations interested in joining the HUB to connect with a statewide network of participants and providers of evidence based programs should contact the Iowa Community Hub (see this link for more information). Individuals, agencies and clinical partners can also directly refer individuals or patients to Walk with Ease directly through the Iowa Community Hub. Individuals that are referred will be contacted to determine interest and eligibility for different versions of Walk with Ease. Direct referrals from physicians can also be facilitated directly through electronic medical records (see this link for more information) |
